Please find below our Club Registration and Medical Form. This form MUST be completed and returned to our Admin Desk, on your first visit to the club. If you don’t have a printer or forget to bring it with you, do not worry, we have spare, blank copies at the Admin Desk. Our Admin Desk is inside the Sports Hall on Court 5.
